- Immunizations / Physical - may take current record to the Health Department and have it transferred to Tennessee.
- Proof of Residence**
In State (1st - 12th grades)
- Proof of Age*
- Immunizations / Physical
- Proof of Residence**
In County
- Proof of Residence**
- Records will be transferred
* Proof of age must be one of the following documents:

- Birth certificate (or an officially acceptable evidence of the child's date of birth)
- Passport
- Legal adoption or custody papers
** Proof of Residency - these items will need a current date.

- Utility statement (gas, electric, phone, etc.)
- Letter of correspondence from government agency
- Letter from landlord verifying the residence
- Lease agreement
If you have a parenting plan or other court documents, please bring a copy with you.
